Required Documents for Registration and Enrollment

  • Electronic Registration via the HEPIQ Application.
  • Preparatory School Certificate authenticated by the General Directorate of Education and addressed to the University of Nineveh / College of Pharmacy, including the verification code (QR Code). (A certified grade statement with a QR Code may be submitted until the official academic document is completed.)
  • Medical examination to ensure the student’s health fitness, which is an essential requirement for completing the registration process.
  • Two copies of official identification documents (Unified National Card, or Civil Status ID Card + Iraqi Nationality Certificate) and the Residence Card.
  • Six recent personal photographs with a white background.
  • Student’s signature on the written undertaking confirming commitment to university laws and regulations.
  • Payment of the prescribed registration fees for the College (the payment is made electronically during registration through the HEPIQ application).
  • Students admitted through the Government Parallel Admission Channel are required to provide a guarantor who is a currently employed staff member. The guarantor must submit the official identification documents (the Unified Identification Card and Employee ID Card), along with a certificate of employment continuity addressed to the University of Nineveh / College of Pharmacy, specifying the nominal and total salary. The guarantor’s personal attendance is required to sign the guarantorship undertaking.

After completing the paper-based registration process and submitting all required documents, the student’s information is entered electronically into the Student Information System (SIS) approved by the Ministry of Higher Education and Scientific Research. This ensures that the student’s academic record is officially documented within the College and maintained throughout all stages of study until graduation. Subsequently, the student is issued a university identification card and becomes officially registered and obligated to attend classes regularly.
